Disciplinary Decision Form

If a conduct concern has been raised and investigated, the results of the investigation will be reviewed at the next Board meeting. The Board will determine the best course of action and will document their decision on the form below.

According to the Conflict Resolution and Disciplinary Procedure, the individual may appeal any such decision.
